Reach Out to Us - Or Follow Your Provider’s Referral!

Getting started is easy. Simply contact us through our website or your healthcare provider, and we’ll help you determine if you are eligible to participate and guide you through the process of joining Era. If you’ve already been referred by your provider, you can also follow their instructions or reach out directly to us at info@erasupports.com.

Initial Assessment

Once you’re enrolled, we’ll set up an initial assessment to understand your medical history, behavioral health needs, and any social challenges you may be facing. This helps us create a personalized care plan tailored just for you.

Meet Your Care Team

Your care team will include your primary care doctor, care managers, behavioral health clinicians, and peers who specialize in substance use recovery. They’ll be by your side throughout your journey, offering regular check-ins and adjusting your care plan as needed.

Access Your Care

Through our app, website, or phone, you can access care at any time. Whether it’s a telehealth visit, connecting with your care team, or educational resources, the support you need is always available. You’ll also have in-person appointments with your provider to ensure comprehensive care.

Ongoing Support and Adjustments

As you progress in your recovery, we’ll be there to provide continuous support. Your care plan will evolve based on your changing needs, with adjustments to medications, therapy, or social services to help keep you on track.

Transition to Independent Management

Our ultimate goal is to empower you to manage your recovery independently. As you reach key milestones, we’ll help you transition to long-term management of your health and well-being.  But also remember, you aren’t alone and Era is there when you need us.

Frequently Asked Questions

Can I sign up for Era without my primary care provider’s involvement?

At Era, we really believe in centering your care around the primary care provider. Who that is may be flexible, depending on who currently manages your overall care, and we can also help you find a suitable provider in your community to support your recovery with our team at Era.

What is my commitment when I join?

Your commitment is to your own recovery and we are here to help. There are no hard requirements or commitments, and you will not be discharged for missing appointments or returning to use, and we don’t have abstinence requirements. The basic care model consists of meeting with your Era care team members for 1-2 hours per month. Depending on complexity of your recovery and other conditions, this may be greater to meet your needs in reaching your goals and getting to the best outcomes for you.

Does it cost me anything to participate in Era?

While Era is available to members with any insurance coverage, depending on your carrier, there may be a small co-pay associated with Era visits. For some plans, including Medicaid, there is no cost to you at all. For specific cost sharing please contact an Era team member.

Will Era clinicians prescribe medications to me?

No, prescriptions will still be delivered and managed by your doctor.

Will Era take over providing all of my healthcare?

No, the Era team partners with your primary care provider as your extended care team. Your doctor will continue to be your main source for most of your physical health needs, including MAT
